The line between lead and support is a thin one. Pigeon-holing lead actors into supporting slots really does a disservice to the true character actors. In the case of Ralph Fiennes in The End of the Affair, he was clearly the protagonist and had the larger part than Julianne Moore whose character was, of course, much more sympathetic. There were, however, three wonderful supporting players in the film: Stephen Rea as the cuckholded husband, Ian Hart as the detective and Sam Bould as the boy, any one of whom could have been slotted into the supporting actor category.
In the case of Al Pacino in The Insider, his was a borderline lead/supporting role that could have fit in either category, though in my opinion his perfromance didn't merit recognitioon in either. Christopher Plummer's uncanny portrayal of Mike Wallace in that film was far more worthy of a supporting nod.
The one really great male performance of 1999 was Richard Farnsworth's in The Straight Story.

I guess for me, some movies can only have one lead, despite the screen time. I saw Ralph as a supporting character to Julianne's story. And what I remember the most from The Insider was Russell Crowe. Keep in mind also for me, this was an extremely weak year for men in general. I despised The Cider House Rules and The Sixth Sense so they garnered no nods from me. Out of that weak lineup in the supporting actor list, Ralph is the clear front-runner. In the lead, Spacey could have been beat by any of them with the exception of Cusack. Moore easily took her supporting win, and barely edged out her win in the lead category.
Barry, I'm only mentioning it because I don't see the logic. Keep in mind, I also happen to think that Haley Joel Osment was a lead in The Sixth Sense so feel free to disregard.
